Objective
Developing a comprehensive Internet-based solution and resource for the Health & Social Welfare industry that provides the ability to enhance and automate organization's enterprise-wide discharge-planning referral and placement process :
- It should automate discharge planning process by connecting hospitals with care homes providers;
- It should automate admission planning process by connecting Social services with care homes providers.
Solution
We created a professional web application that consolidates all of the necessary information that an acute care discharge planner, placement manager and Social workers require to make an informed decision on the best post-acute options for the patient/appropriate placement for the client. It provides a secure communication link between discharge planner, placement manager, Social workers and care providers that automates the referral and transfer process. Care Homes Locator allows care providers to automatically receive and manage patient referrals from hospital and residents referrals from Social services. It provides real-time web marketing reports and allows the care provider to discharge to step-down units. Platforms, Tools, and Technologies
